Returns Policy

Terms
2% 30, net 31 days from invoice date. Payment must be received by Stratus
Pharmaceuticals, Inc. within 30 days of invoice date in order to be eligible for 2% discount.
• 1 1/2% service charge will be added to unpaid balance after 45 days.
• A minimum order of $150.00 is required. The minimum for shipment on all products are 12 units per line. shipping and handling will be charged to all orders not following these terms. Minimum s & h charge: $5.00. this does not apply on shipment of backorders, automatic shipments of new products, and shipments of merchandise exchanges.

Returned goods
• Any unopened, original trade units with less than six (6) months past expiration date from original purchaser.
• Any transit damage should be reported to stratus within ten days of receiving shipment to be eligible for credit.
• Merchandise may be accepted for credit or exchange until six months after expiration date.
• Written requests including description, quantity, ndc number, and lot number should be faxed to:

• All returned goods must be approved by a Stratus customer service representative through

Additional information
Returns are only accepted through the wholesaler. All returns are subject to inspection of original purchase documents and credit will be based upon purchase price net of discount. No credit will be allowed for processing fees. Credit will only be issued for goods authorized and received.

Return Policy Authorization:
Contact a Stratus representative or Stratus Pharmaceuticals Customer Service at
800-442-7882 for assistance with any Return Goods authorizations.

Returnable Items:
• Expired products or excess inventory less than 6 months past expiration.
• Unopened original trade units less than 6 months past expiration.
• Products with transit damage. Any transit damage should be reported to Stratus within ten days of receiving shipment to be eligible for credit.

Non Returnable Items:
• Items more than 6 months past expiration date.
• No credit will be issued for items that have been involved in a sacrifice, fire, bankruptcy sale or items damaged by fire, water or smoke.
• Items expressly sold on a non-returnable basis.
• No credit will be issued if proof of purchase cannot be verified.
• No credit will issued for products repackaged or in other than Stratus containers
• Items obtained in violation of state and federal law.

Procedure for Returning Items:
Written requests including description, quantity, NDC number and lot number should be mailed or faxed to:

All returned goods must be approved by a Stratus customer service representative and require an authorization code to be accepted for credit.

Terms of Return Policy:
Stratus retains the right to establish the value of the returned goods based on the original outgoing price. All returns are subject to inspection of original purchase documents and credit will be based upon purchase price net of discount. Product returned with more than 3 months expiration dating remaining are subject to a 10% re-stocking
charge. Merchandise may be accepted for credit or exchange until six months after expiration
date. Credit will only be issued for goods authorized and received.

Company Disclaimers:

Products returned are subject to Stratus evaluation at time of receipt. Stratus reserves
the right to destroy returned products which Stratus feels are unfit or unsafe for use.
